Título:Single lesion subpolar lepromatous leprosy and its possible mode of origin.

Resumo:Three case reports of patients with a single, nodular, subpolar lepromatous skin lesion, one on the left elbow, another on the posterior aspect of the left leg, and the third on the extensor ulnar aspect of the right forearm, are presented. The lesions, clinically and histopathologically, resemble lepromas which develop at the site of experimental inoculation of Mycobacterium leprae in armadillos. These are sites on the body which are likely to be traumatized. With the distinct possibility of the presence of viable M. leprae in the soil of Louisiana and Texas from wild armadillos with the natural disease, it is suggested that these three patients acquired the infection from the environment and had inoculation lepromas^ien.

Título:An electron microscopic study of lymphatics in the dermal lesions of human leprosy.

Resumo:The dermal lymphatic vessels in lepromatous and tuberculoid leprosy lesions were studied by light- and electron-microscopy. In the lepromatous patient, lymphatic vessels were seen in both intra- and peri-granulomatous areas. The lymphatic lining cells contained lipid droplets, lysosomes, and numerous pinocytotic vesicles. Cells bearing bacilli were only occasionally seen. In the tuberculoid cases, lymphatic vessels were seen only along the edges of the granulomas and the lining cells were less prominent. Inflammatory cells, both lymphocytes and histiocytes, were found traversing the walls of lymphatic vessels in both groups of patients. The results of the study confirm the continued and increased functioning of the lymphatic drainage system in dermal leprosy lesions, and indicates that it may be a major route for the clearance of lipids from the lipid-rich bacilliferous lesions in the lepromatous patient. The lymphatic pathway appears to be a minor pathway for the dissemination of Mycobacterium leprae in comparison with the blood vascular system^ien.

Título:Reactive oxygen intermediates inactive Mycobacterium leprae in the phagocytes from human peripheral blood.

Resumo:Reactive oxygen intermediates such as hydrogen peroxide, superoxide, and hydroxyl radicals are important microbicidal components, and they could also play a role in an infection with Mycobacterium leprae. A comparative study of the level of hydrogen peroxide and superoxide produced by peripheral blood phagocytes from normal healthy individuals and lepromatous leprosy patients showed a deficiency in superoxide production in the patients. In the phagocytes from normal healthy individuals, there was good release of superoxide ions, and this mediated the killing of M. leprae. The lack of superoxide production allowed the viability of M. leprae inside the macrophages from leprosy patients. This deficiency could be rectified by the use of an immunomodulator, the delipidified cell wall of M. leprae. This modulation resulted in the ability of the patients' phagocytes to respond to M. leprae, to produce reactive oxygen intermediates such as superoxide, and also to kill the bacteria. These observations indicate that delipidified cell wall could have significant potential to positively modulate the immune-deficient cells of leprosy patients.^ien.

Título:Results of a modified WHO regimen in highly bacilliferous BL/LL patients.

Resumo: regimen consisting of 600 mg of rifampin once a month, 100 mg of clofazimine on alternate days, and 100 mg of dapsone daily was used in 56 untreated, highly bacillated borderline lepromatous/lepromatous (BL/LL) patients with an average bacterial index (BI) of 4.45. Treatment was continued until skin-smear negativity. After 2 years of therapy, none of the patients had become smear negative and the average BI was 2.56. There was no growth on inoculation of skin-tissue biopsies in the normal mouse foot pad after 6 months of therapy. Bacillemia was still detectable in 11/50 patients, and significant ATP levels were detected in Mycobacterium leprae from skin-tissue biopsies in 16% of the cases. After 3 years of therapy, three patients had become smear negative. The average BI was 1.30. None of the patients had detectable bacillemia, and 5% of the cases showed detectable ATP levels in M. leprae from tissue biopsies. After 4 years of therapy, 41.7% of the patients had become smear negative. The average BI was 0.66, and no ATP was detected in any of the purified bacillary suspensions. The fall in BI was accelerated, and more patients on continued treatment became negative earlier compared to those having treatment for a limited duration, as reported by others^ien.

Resumo:(Role of temperature, microtraumatisms by elongation or subluxation and canalar stricture.) Cold has a slight aggravating effect on leprosy which is observed in some countries as Iran, South Russia, North India and the Andean cordillera. The subluxation of the ulnar nerve has facilitating effect in the development of the neuritic damage in a limited number of patients. The elongation is important for the production of the ulnar neuritis. The canalar stricture is a major factor causing nerve damage in leprosy. It explains the segmentary localization of the principal damages of the leprous nerves in the proximal areas situated above the tunnels. Intraneural hypertension appears first and is responsible for the swelling of the nerve which gets entraped inside the tunnel. A circulus viciosus is, then, created which maintains and aggravates the intraneural hypertension.

Resumo:A brief review of the contradictory opinions of different authors as to the nature and the place of tuberculoid cases of leprosy in the evolution and pathogenesis of the disease is presented. Mention is also made of their failure to agree on what could be generally considered as the esential histological features of this condition. A case of tuberculoid leprosy which has undergone transformation into a lepromatous-type case within six years of observation is presented. This case report together with the clinical, bacteriological and histological studies of other authors seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. That such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones or benign infections, is indicated. The need for continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Resumo:Rheumatic manifestations are a common feature of leprosy. The present study was performed to assess the incidence of rheumatic manifestations in leprosy. Methods: seventy cases of leprosy attending the indoor and outdoor departments were studied. Results: Out of the 70 cases studied, rheumatic manifestations were seen in 61.42% of cases: arthritis in 54.28% and soft tissue rheumatism in 17.14%. Enthesitis was seen in 2.84% of cases. The incidence of arthritis in lepra reactions was 57.57%. Conclusions: Musculoskeletal manifestations can occur at any time during the infection. Articular inflammation in leprosy usually occurs in reactive states, particularly erythema nodosum leprosum (ENL). Leprosy can present with arthritis as the first symptom.

Resumo:Some of the possible factors involved in the genesis of amyloidosis during the course of lepromatous leprosy have been studied. In a group of 101 patients from the U. S. Public Health Service Hospital at Carville, La., it is estimated that 40 to 50% have concomitant amyloidosis. This high proportion has been previously wll-documented by autopsy studies (2,3) and was suggested by a 31% positive result in 69 gingival biopsies among this group. Congo red results were often positive, particularly when accompanied by persistent albuminuria. The high percentage of amyloidosis at Carville lies in distict contrast to that of 119 patients studied at Guadalajara, Mexico, where amyloid was diagnosed in only 6%. Factors which may play some role in this different include discrepancies in animal fat consumption between the two population groups. Patients at Carville eat fewer calories but twice the percentage of saturated fat in their diet as compared to the Mexican group studied...AU.

Resumo:The reactional states seen in lepromatous form (erythema nodosum, leprosum, exacerbation) and i interpolar forms (tuberculoid exacerbation and tuberculoid reversal reaction) result in cutaneos, nervous, lymphatic, visceral and polyarthritic acute manifestations. The sucessif relapses result in articular, cutaneous and muscular sequelae of the hand but a paralysis of medial and ulnar nerves, often associated with leprosy, make them different from rheumatoid arthritis. They can be summarized thus: 1- Fixed claws hand associated with stiffness, erosions and sometimes subluxations of interphalangeal joints, mainly proximal and metacarpophalangeal joints...AU.

Resumo:A hanseníase é uma doença infecciosa crônica causada pelo Mycobacterium leprae e que ainda apresenta altas taxas de prevalência nas regiões norte, nordeste e centro-oeste do Brasil. Atualmente, o esquema poliquimioterápico (PQT) preconizado para os pacientes multibacilares tem duração de 12 meses; após o término da última dose esses pacientes recebem alta, sem a necessidade de qualquer avaliação bacteriológica. No presente estudo avaliou-se os índices baciloscópico (IB) e morfológico (IM) de 73 pacientes multibacilares, no momento do diagnóstico e ao final de 12 doses da PQT. Após o término das 12 doses, o IM foi positivo em 20,54% deles, sendo todos virchovianos; esses pacientes continuaram a receber a medicação até 24 meses, persistindo a positividade em 4,11% deles; estes continuaram com a medicação até a negativação do IM. Neste sentido, os autores sugerem que os IB e IM deveriam ser realizados ao final do tratamento e, nos casos de positividade do IM, os pacientes poderiam continuar recebendo a medicação por 24 meses ou mais, evitando, assim, possíveis recidivas e transmissão. A mesma recomendação é feita para pacientes portadores de alguma imunodeficiência. (AU).

Título:Cellular response to the intradermal injection of recombinant human gamma-interferon in lepromatous leprosy patients

Resumo:The local response to single intradermal injection of 10 ug recombinat gamma-interferon (rIFNgamma) has been studied in 17 patients with lepromatous leprosy. Of these, 2 patients additionally received two intradermal injections of 10 ug rIFNgamma at received another site. The results were compared with those of 3 patients who received three injections of the same dose at a single site in an earlier study. One to 7 days after lymphokine administration 4-mm pinch biopsies were obtained and axamined for cellular alterations in the dermis and epidermis. This allowed a kinetic analysis of mononuclear cell infiltration, keratinocyte proliferation and differentiation and Langerhans cell redistribution. At 24 hours, the migration of large numbers of helper T cells and monocytes was already prominent and associated with induration. Mononuclear cell eccumulation peaked at 72 hours but then persisted for 5-7 days. Only smal numbers (one-third or less of toal T cells) of suppressor/cytotoxic T cells were present at any time, and granulocytes were absent. Two daily injections of rIFNgamma led to a more intense accumulation of cells. Ten ug of rIFNgamma resulted in enhanced keratinocyte proliferation, Ia expression, and thickening of the epidermis. At 24-48 hours major histocompatibility Class II (Ia) antigen was first noted on the dividing cells of the basal layer. By 72-96 hours the entire epidemir exhibited strong expression of Ia antigen on cell surfaces. Repeated doses of lymphokine accentuated these changes and resulted in a more prompt keratinization and sloughing of this layer. Whereas a single dose of rIFNgamma resulted in the upward movement of T6+ Langerhans cells (LCs) in the epidermis, two injections led to a 50% reduction in their numbers and three doses were associated with an almost total loss of detectable T6+ LCs from the epidermis. These are probably aloughed along with keratinocytes. In contrast to the situation with a delayed immune response in the skin (purified protein derivative), no LCs accumulated in the dermis in association with helper T cells (AU).

Resumo:The relative recurrence risk ratio lambdaR (and particularly the sibling recurrence risk ratio, lambdaS) is often of interest to those wanting to quantify the genetic contribution towards risk of disease or to discriminate between different genetic models. However, estimating lambdaR for complex diseases for which genetic and environmental risk factors are both involved is not straightforward. Ignoring environmental factors may lead to inflated estimates of lambdaR. We present a marginal model which uses a copula function to model the association in cumulative incidence rates between pairs of relatives. This model is applicable to present-state data and allows estimation of risk of disease in a pair of relatives (and hence lambdaR), given measured environmental covariates. We apply the model to leprosy among sibling pairs from the Karonga district, Malawi. If risk factors are ignored, the apparent lambdaS in this population is over 3. Accounting for known nongenetic risk factors reduces it to just under 2. (AU).

Resumo:Interrelationship among six patient characteristics recorded upon entry the trial were analyzed for 67 patients with lepromatous and near-lepromatous leproay admitted into two chemotherapy trials. Sex was found to be significantly associated with age and with the histopathologic classification; disproportionately large numbers of older patients and of patients classified as borderline-lepromatous (BL) were males. Classifications of the disease process by clinical and histopathologic criteria were closely associated, but many patients classified BL on histopathological grounds were classified fully lepromatous by the clinical criteria. Measurments of the number of Mycobacterium leprae in the patients made by there methods were also significantly correlated. No significant correlations were found between either classification of the disease process on the hand, and any of the measurments of the numbers of organisms on the other (AU).

Título:The histopathology of lepromatous leprosy in the nose

Resumo:On the basis of clinical, histological and bacteriological assessments, 31 patients in Central India were selected and classified as having active but early lepromatous leprosy and 4 patients as having early borderline leprosy. From the nose of each patient an average of 4 biopsies were taken from particular sites of the septum and turbinates either by punch biopsy or dissection with a scalpel. The nasal tissues from all the lepromatous patients contained many acid-fast bacilli; no bacilli or abnormalities were seen in the nasal tissues from the borderline patients. The histopathology of these highly bacilliferous tissues is described. Bacilli were universally seen in macrophages , but they were also seen in blood monocytes and polymorphs, fibroblasts, squamous and pseudo-columnar epithelium, kerati, peri- and endo-neurial cells of tiny nerve bundles, erectile tissue, vascular plain muscle, perivascular histiocytes and frequently and abundantly in the cytoplasm of endothelial lining cells of lymphatics and of small blood vessels and free within the lumina of these vessels. Five basic mechanisms of scape of bacilli from the sunbmucosa on to the surface, and thus into the external environment, are described. Secondary infection, in the presence of an expansile lepromatous infiltrate, together with simple trauma to the surface epithelium, are the main factors in the discharge of bacilli. These histopathological observations are consistent with the findings from other recent studies on the nose in leprosy regarding (1) the large mumbers of morphologically intact and viable Myco. leprae excreted in the nasal mucus of lepromatous patients; (2) the clinical changes observed in the nose of such patients, and (3) the similar nasal involvement and excretion of Myco. leprae from the nose of mice inoculated with leprosy bacilli of human origen. On particular interest was the frequency and intensity of bacilli within the endothelial lining cells of small blood and lymph vessels and the presence of bacilli free within the lumina of these vessels or within monocytes and polymorphs. The possible dynamic significance of these observations in the pathogenesis of leprosy is discussed. The significance of all these observations in relation to (1) the spresd of leprosy; (2) local factors in the nose which might favour the growth of Mico. leprae, and (3) the nose as a portal of entry, are discussed.
